  • When a child is born in Ontario, their birth needs to be registered within 30 days with the Government of Ontario. For some parents, registering a birth within 30 days is not always possible. In these cases, if you’re eligible you can still register a birth online for free, within 12 months of the birth date. The process to register a birth after 12 months is longer and costs $50.

    Registering a birth in Ontario usually is a two-step process:

    1. A Statement of Live Birth Form is completed by parents of newborns and the parents submit to ServiceOntario

    2. A Notice of Live Birth Form is completed by qualified medical staff and these staff submit to ServiceOntario

    It’s quicker and easier for parents to register a birth online than to submit a paper Statement of Live Birth form by mail. If you register a birth by mail, you will need to apply for your child’s documents separately.

    In one application, you can:

    1. Register your child’s birth

    2. Apply for a birth certificate

    3. Apply for a Social Insurance Number (SIN)

    4. Apply for Canada child benefits (including the Ontario-specific child benefits)

    5. Request an Education Savings Referral

  • An infant is eligible for the Ontario Health Insurance Plan (OHIP) if they:

    • are born in Ontario

    • have at least one parent or guardian who is a resident of Ontario and

    • will be staying in the province for 153 days in the following year

    In order to register your newborn with OHIP, you must complete an Ontario Health Coverage Infant Registration form. Hospital staff or a registered midwife will give you this form soon after your child is born.

    For this form, you will need to:

    • Provide your child’s name, birth date and mailing address.

    • Confirm that your child’s primary place of residence will be Ontario.

    • Confirm that your child will be physically present in Ontario for at least 153 days in any 12-month period.

    Detach the bottom of the form and keep it for your records. It contains the Health Card (OHIP) number assigned to your child.

    You can use this portion of the form to access health services for your child until you receive a physical, plastic OHIP card for your child by mail.

  • The Infant Hearing Program offers free hearing screening for babies living in Ontario. Hearing screens are provided until two months of age. This service is provided under the Ontario Infant Hearing Program which is funded by the Ministry of Children, Community and Social Services.

    If you have any questions or would like to arrange an appointment to apply in person, contact Toronto Indigenous Services Canada directly at 416-973-5282. They are located at 655 Bay St Suite 301 and service hours are by appointment only Monday-Friday, 8:30am to 3:30pm.

    If your baby requires coverage for medications and you have yet to apply or receive their status card, the baby can be put temporarily under the parent’s status card for coverage. In order to do so, the parent needs to contact the non-insured health benefits drug acceptance centre for approval at 1-800-580-0950. You will be requires to provide documentation of birth which your midwife will provide to you.
